Introduction

For those traveling to remote areas (i.e. deserts, rainforests, tundra, or simply areas without developed state infrastructure), planning for medical emergencies is essential. However, Remote Medical Evacuation (RME) carries distinct operational risks. This blog post utilizes Ashley Judd as a case study for the logistical challenges associated with RMEs. Judd, an American actress, traveled to the Democratic Republic of the Congo’s Equateur region for humanitarian purposes in early 2021. On February 3rd, she tripped while traversing the rainforest and broke her leg. The ensuing RME to South Africa took over 55 hours in total. During the ordeal, she reportedly faced critical risks including nerve damage and internal bleeding (1).

Ashley Judd’s Evacuation
sections[0].body
In January 2021, Judd travelled to the Democratic Republic of the Congo to observe Bonobos in the Equateur region. On the early morning of February 3rd, she tripped over a tree and broke her leg in four places. To arrive at her final point of extraction, she first needed to reach her campsite at Djolu. Judd was then airlifted nearly 1,000 kilometers from Djolu to Kinshasha, which had the infrastructure needed to stabilize her condition before her Medical Evacuation to an adequate facility in South Africa.

The Risks of RMEs
sections[0].body
Judd’s story highlights the risks associated with RMEs. Namely, remote zones can lack the infrastructure needed to conduct extractions via land, sea, or air (a). First and foremost, sea evacuations are bound by access to bodies of water and the ability to dock extraction vessels near the site of an accident. Environmental factors take on a degree of importance, as a narrower body of water or adverse weather may restrict the Medical Evacuation service from utilizing larger, more well-equipped vessels or, in rare cases, force them to delay or forgo maritime rescue entirely (2). Case in point, the US Department of State has identified navigability and access to medical infrastructure within the DRC – which has river connections to the Atlantic Ocean – as key travel risks (3). Air evacuations are similarly bound by access to a suitable landing area and weather conditions (4). Finally, ground evacuation can require access to adequate roads or otherwise necessitate travel through poor road infrastructure (b). As Judd’s story shows, ground evacuation on poorly paved roads and through remote areas can be long and painful (5) (c). Overall, these conditions mean that RMEs can take hours or even days, with potential for further logistical complications.

Endnotes

a. Regulations on RMEs and data on response times occasionally mix military and civilian contexts. As such, this section relies, in part, on military studies. It should be noted that these studies are not used to reflect exact response times, but rather to provide a general idea of the logistical challenges facing RMEs.
b. In her post on Instagram following the incident, Judd describes riding on the back of a motorcycle through dirt roads for roughly six hours while injured (1).
c. These conditions may increase the risks of the mission. See section 4.7 of the US Coast Guard Manual for more information (2).
